By checking this box, I'm confirming to have read the Participation Enrollment Agreement in its entirety, accept its policies, and acknowledge the following:

I consent to being video recorded with my fellow classmates by JST Coaching & Training. I understand that this consent is perpetual, that I may not revoke it and that it is binding.
If I DO NOT CONSENT to being video recorded by JST Coaching & Training. Therefore, I will be responsible for turning off my camera during all training classes.
